All new patients are asked to complete a New Patient Packet. These forms are available online and for pick-up in person at any of our clinics. We will also mail these upon request.
If you are making an appointment for a minor child, complete the new patient packet with their information.

If you are transferring care, this gives Konza permission to receive your medical records from another clinic.
If you don’t have insurance or can’t afford your insurance copayments, we can help. You may be able to pay less for medical, dental, and behavioral health depending on your household size and income. Learn more about our Discounted Fees here.
You can complete the Discount Fee Application online. You can also pick up a copy of this application in person at any of our clinics.
If you don’t qualify for a Discount Fee, you can save by paying directly when you receive services. Payment plans are available.
