Call to Make an Appointment
Complete a
New Patient Packet
All new patients are asked to complete a New Patient Packet. These forms are available online and for pick-up in person at any of our clinics. We will also mail these upon request.
If you are making an appointment for a minor child, complete the new patient packet with their information.

New Patient Packet:
Basic information your provider needs to know about you and your health history.
Authorization to Release Information:
If you are transferring care from another provider, this gives Konza permission to receive your medical records from another clinic.
Discount Fee Application:
If you don’t have insurance or can’t afford your insurance copayments, we can help. You may be able to pay less for medical, dental, and behavioral health depending on your household size and income. Learn more about our Discounted Fees here.
If you don’t qualify for a Discount Fee, you can save by paying directly when you receive services. Payment plans are available.
Things to Bring to Your First Appointment:
- Insurance card
- Proof of Income (if you are applying for discounted services)
- A list of all medications you are taking (or bring the medications with you), and your vaccination history, if you have it
- Your New Patient Information forms (if not already completed online)
