Diabetes Education & Support

Konza dietitians provide nutritional support for anyone living with diabetes.

Konza dietitians can provide the Healthy Food Prescription Program in partnership with Live Well Geary County. This prescription is written by a Medical or Nutrition provider for patients with diet-related health risks.

Participants in the Healthy Food Prescription Program are eligible for FREE nutrition education and $50 dollars to purchase fruits and vegetables, redeemed at the local farmers markets or Konza Prairie Community Health Center.

During nutrition appointments you will spend 30-60 minutes working with a dietitian to discuss nutrition challenges, medical history, and create goals.
