Konza Prairie Community Health Center offers low-cost sports physicals year-round for $25 or a donation of five cans of food.
These services are available at Konza clinics in Junction City and Manhattan.
Appointments are required and are open to all youth who need a sports physical to participate in athletics.
A sports physical can also be included with a child’s annual wellness visit, which is a comprehensive health check up.

Please complete the Student/Parent portion of the KSHSAA Pre-Participation Physical Evaluation form before your appointment.
These forms are available at the clinic and online in English and Spanish.
During the sports physical, a Konza medical provider will:
- Review the athlete’s health history
- Review immunizations to ensure they are up-to-date
- Provide a sports physical exam to determine if the athlete can safely participate in their chosen activities
All canned food will be donated to the Konza Cupboard food pantries located within Konza clinics.
