Community Outreach

Community Health Workers help anyone in the community and they can meet you where you are.

Konza Community Health Workers support and guide patients and families to connect with additional supportive programs and services. They focus on benefits enrollment, social service navigation, community outreach, and administering patient-centered education and programming.

There is no cost for their services and you do not need to be a Konza patient.
