Konza Prairie Community Health Center is offering low-cost sports physicals, now through August, at the Junction City clinic – located at 361 Grant Ave – for $25 or a donation of five cans of food.
Appointments are required and are open to all youth who need a sports physical to participate in athletics.
A sports physical can also be included with a child’s annual wellness visit, which is a comprehensive health check up.

Please complete the Student/Parent portion of the KSHSAA Pre-Participation Physical Evaluation form before your appointment.
These forms are available at the clinic and online in English and Spanish.
During the sports physical, a Konza medical provider will:
- Review the athlete’s health history
- Review immunizations to ensure they are up-to-date
- Provide a sports physical exam to determine if the athlete can safely participate in their chosen activities
“All school athletes are required to get an annual sports physical before they may participate. We want to expand community access for healthcare overall, providing reduced price sport physicals is just one way to do that,” said Danielle Hallgren, CEO of Konza Prairie Community Health Center. “We want to make the process easy for families so kids can get out there and play.”
All canned food will be donated to the Konza Cupboard – a free food pantry located at the Junction City clinic.
